    Halifax Health is seeking a Care Manager for the Regional Oncology Center - Oncology Services.


    The Oncology Care Manager is a designated member of the Oncology team who performs case management, including coordinating the psychosocial needs of outpatient oncology patients and their families, assisting with post-therapy planning that will connect social services and post-therapy planning that will provide appropriate utilization review for treatment and appropriate care for outpatient oncology patients.


    Job Qualifications

    • Formal Education/Current Florida licensure: RN, LPN, Social Work, or other social services related field (Masters preferred).
    • Highly developed assessment and interpersonal communication skills.
    • Abilities to organize, prioritize, analyze, design, and implement plans.
    • Ability to handle multiple responsibilities and tasks in stressful situations.

    Job Duties and Responsibilities

    • Screen patients referred by physician and/or staff and identify patients who require social services or utilization review for treatment.
    • Review medical records with respect to established criteria for severity of illness and intensity of service and initiate appropriate interventions.
    • Communicate with attending physician and/or physician advisor as medication utilization and other concerns are identified.
    • Communicate utilization and planning information with insurance and/or review companies as needed.
    • Conduct interviews with patients, significant others, and appropriate staff to assess the patient’s concerns in coping with health care needs.
    • Assess patients with respect to health care needs, living situation, cognitive and physical functional abilities, support systems and resources.
    • Assist patients, significant others, and physicians in identifying health and social resources needed to assure quality care.
    • Advise and consult with physicians and staff regarding the psycho-social needs of the patient and significant others.
    • Document and compile statistical information of assessments and interventions.
    • Generate referrals to address patient needs with reference, but not limited to:  financial assistance, medication and medical follow-up, transportation, medical equipment and supplies, home health care, nutrition, substance abuse, rehabilitation, and community resources.
    • Provide case management to outpatient oncology patients.
    • Solve related problems to all areas of patient care as required.
    • Provide leadership and guidance.
    • Handle multiple responsibilities and tasks in stressful situations.
    • Report to Cancer Committee for Commission on Cancer and Distress Screening purposes.
    • Other duties as assigned.
