Overview

Halifax Health is seeking a Case Management Coordinator.

Summary

The Case Coordinator oversees a mix of clinical, operational, and business activities.  Works closely with care management team to ensure seamless transitions of care for our patients.

 

Job Qualifications

  • BSN or BS in Health Related Field
  • Five (5) years acute care hospital experience
  • Current clinical knowledge and experience, particularly with the senior population
  • Experience/knowledge of moving patients from acute to post-acute settings throughout the care continuum
  • A passion for improving healthcare
  • Demonstrating the ability to lead and manage complex projects that are strategic in nature and scope
  • The ability to collaborate and motivate other healthcare providers
  • Strong/effective problem solving skills
  • Experience with performance measurement, outcome analysis, monitoring tools; computer skills
  • Excellent oral and written skills
  • Excellent organizational skills with demonstrated ability to complete projects timely and within budget

Job Duties and Responsibilities

  • Supervisor of Care Managers.
  • This role is responsible for achieving the established targets for Episode Payment Models (EPM) including Bundle Payment, Mandatory Bundles as well as contributing to other value based care. Establishes an operational plan for service based upon mission and strategies of the post-acute care program initiatives. Develops understanding of all value-based payer program requirements including CMS. Works closely with Legal and Compliance meeting regulatory compliance for all initiatives.
  • Develops strong working relationships with the post-acute care team, including those that facilitate patient care across the care continuum.
  • Identifies and implements specific program goals including models of care redesign efforts required to improve performance in the following areas: Optimal next site of care, Skilled Nursing Facility (SNF) utilization and selection; SNF Length of stay (LOS); readmissions reduction and other desired outcomes pursuant to targeted goals. (consider including Inpatient Rehab and Home Health utilization)
  • Create monthly dashboards, readmission reports, quarterly and other reports as needed to assist with the identification of opportunities for improvement.
  • Develops; revises; reviews as needed all policies and procedures for the operations of the program.
