Overview

Halifax Health is seeking an individual for Utilization Review in the Adult Behavioral Health service line.

Summary

The Psych. ED CM identifies and coordinates treatment of patients who meet criteria of Grant Funding HMC Psych. Services receives in collaboration with SMA. The CM will assess, coordinate, and refer identified patients who enter into HMC's Psych. ED with the goal to expedite throughput and decrease Psych. ED and/or Inpatient service utilization. To facilitate an immediate connection/referral to SMA's embedded Care Coordinators in Adult Psych. Department.

Job Qualifications

  • Completion of an accredited LPN or RN nursing program, BSN prefered
  • Case management, care coordination and/or discharge planning experience preferred.
  • Experience working with Substance Abuse and Mental Health clients, or other relevant human service field.
  • Licensed Nurse in the State of Florida
  • Demonstrates effective interpersonal and communication skills
  • Demonstrates flexibility via an ability to adapt to changing priorities
  • Demonstrates good customer relations
  • Ability to prioritize assignments and effective time-management skills
  • Basic knowledge of clinical and psychosocial aspects of patient care
  • Must be detail oriented, flexible, and committed to patient advocacy
  • Demonstrates skills in planning, organizing, and managing multiple functions and complex processes
  • Excellent verbal and written communication skills required
  • Knowledge of basic computer software programs
  • Knowledge of area community resources and referrals
