Overview

Halifax Health - Hospice is seeking a Continuing Care Liaison for the Access Team.

Summary

The Continuing Care Hospital Liaison promotes Halifax Health's health services (home health, hospice, rehab and physician services) in the community through educational inservices and liaison with physicians’ offices, health care facililties and community groups. Provides education regarding continium of care services (i.e. hospice, home health).  

 

 

Job Qualifications

  • Bachelor's degree (or higher) in related field or equivalent years of experience
  • Knowledge of Hospice and Home Healthcare philosophy of care, Medicare conditions of participation, State of Florida regulations governing hospice and home healthcare, hospice and home healthcare operations and sales and marketing
  • Healthcare experience, medical sales and marketing experience preferred
  • Good written, verbal and public speaking skills
  • Possession of valid Florida driver license and clear driving record

 

Job Duties and Responsibilities

  • Establish and maintain good working relationships with referral sources to include, but not be limited to: physician, hospital case manager, nursing home staff, assisted living facility staff and other allied healthcare professionals.
  • Educate and be an ongoing resource of skilled care and end-of-life care and services offered by Halifax Health-Hospice and Halifax Care At Home to the medical and non-medical community.
  • Educate and be an ongoing resource of Halifax Health system services.
  • Develop plans of care for referral sources based upon referral practices and identify barriers to early referral. Develop and maintain good customer knowledge base.
  • Act as liaison between referral sources and clinical operations of health system. Provide feedback to ensure optimal customer satisfaction and patient outcomes.
